The Quality Improvement Organization (QIO) Program launched a new quarterly learning series that offers healthcare providers and partners nationwide the opportunity to participate in virtual training events focused on healthcare quality improvement.The next virtual training session, entitled “Advancing Effective Communication and Coordination of Care,” will take place on Tuesday, May 3, 2016 from 3:00-4:00 p.m. ET. During this webinar, you will hear from the Centers for Medicare & Medicaid Services (CMS), Telligen, and the Southwestern Pennsylvania Area Agency on Aging as they describe best practices and key elements to help you succeed in achieving effective communication and coordination of care. Additionally, you will hear how the Southwestern Pennsylvania Area Agency on Aging identified and addressed needs for community and social support and how they leveraged partnerships in order to achieve change.
