  1. QAPI Cover Letter: This document presents an overview of HQI’s QAPI Toolkit. You can print the letter to include with your toolkit and to share with your team.
  2. QAPI Written Plan How-to Guide: Get ahead of the curve and be prepared to meet the new Centers for Medicare & Medicaid Services (CMS) Reform of Requirements for Long Term Care Facilities by developing your written plan. Use this step-by-step guide to move through the process. Relevant nursing home operations examples are provided to help guide the conversation.
  3. QAPI Template: This customizable template, available in Word or PDF format, allows you to work as a team to develop a QAPI Plan and easily insert leadership-approved content. The sidebar comments cue the team’s actions through each section as you design your facility specific, data-driven QAPI system.   4. QAPI at a Glance: This resource is perfect as an educational tool to evaluate and solidify staff comprehension on QAPI basics, as well as to build team spirit and instill confidence.
