This webinar is geared to practices, practice facilitators, and others who help practices improve health care delivery, and decision makers who are interested in learning more about how practices can deliver team-based care that is patient centered. The webinar will offer concrete strategies that practices can consider to help adopt a patient-centered approach to planning and delivering team-based care.
