Virginia Hospitals and Partners “Sprinting to Success” at Recent Workshop
On July 18, Health Quality Innovators (HQI) partnered with the Virginia Hospital & Healthcare Association (VHHA) to host a special in-person workshop with Amy Boutwell, MD, MPP, a recognized expert on care transitions. The workshop is part of HQI’s Summer Sprint to Success: ED Strategies to Reduce Readmissions series designed to support Virginia hospitals and partners with improvement efforts targeting multi-visit patients. The day’s agenda involved hospitals and their local partners engaging in dynamic discussions for the development of targeted one-week, 30-day and 90-day action plans.
The “Summer Sprint” coincides with the rollout of the new Virginia Emergency Department Care Coordination Program. Click here for some fast facts.
