For Patients and Families

Your experiences in the healthcare system are important to us. When you partner with your doctor and care team, share in decision-making and tell them your preferences, you have a better chance of staying healthier longer. At HQI we are guided by what we learn from patient representatives in our quality improvement network and we include the voices of patients in our efforts to improve quality and safety in Maryland and Virginia.

Ask Me 3 is a good place to start. A short video introduces the ideas and encourages patients and families to ask their nurse, doctor or pharmacist the following three questions:

  1. What is my main problem?
  2. What do I need to do?
  3. Why is it important for me to do this?

The Universal Patient Compact helps clarify the roles and commitments of patient and providers in a handy one-page flyer.

For Providers and Stakeholders

We also work in the community, with providers in all care settings, state and local stakeholders, and patient groups to help providers ensure that patients are actively engaged in managing their health as partners. The following tools can help you with a variety of challenges, from preparing for a doctor visit, managing heart health and fining help with end-of-life planning.
